I'm having problems with my Blog listing. It shows results on the initial index.php page, but as soon as i click my pagination links to view results on page 2 or 3 etc it says in the Meta Title "Page Not Found".

My blog posts are set to all display from /blog

BUT, if i choose a Category first, then click the pagination links, all works fine.

What would i need to change to rectify this issue?

My permalinks are set to Month and Name. Category Base is set to 'blog' Tag Base is set to 'blog'

The loop code in index.php, archive.php and category.php is this :
