adding Adding a querystring to an image urlURL when clicking 'insert into post'?

I have written a plugin which does some fancy stuff with GD lib when you upload an image. Everything

Everything is working fine, however I need to get the Imageimage title and add it to the image urlURL (and file name of the image) when the user clicks 'insert into post'. the reason is that I am writing the title into the image, it's quite important I do it this way but cantcan't find any refsreferences which could help me?.
