comment WPDB Placeholders and second argument for prepared statements
Please, stay professional: No uppercase only "shouting" here. Aside from that: You posted some code, but not any description what is happening and what your output is, etc. Sorry but at this point I am out.

Not that this would answer both the questions that I have asked you. Now I updated the answer and you will get some debug output. File an edit to your question with that var_dump() output, then ping me again. Also do not ignore @birgire answer to this question. There are some valuable points in there that you should take into account.

Your problem (aside from really hard to read formatting) is that you have a very, very hasty style of writing PHP. You do not care about key names, not about assigning variables no matter if you need them or not, etc. Please get someone who will review your code. This site is not code reviews and you clearly need a second pair of eyes and some more in depth understanding of PHP and WordPress plugins/filters API.

After asking that many questions as you did, I assume that you know that recommendations are off topic – you surely have read that in the help center already. About the menu: Your best option is to use a custom template for that.

@Michael I explained and showed you in the answer: There are placeholders. Please read the answer in detail – and do NOT forget to sanitize any arguments/input#.

comment Uninstall, Activate, Deactivate a plugin: typical features & how-to
@J.D. I did not know about that bulk uninstall error there. Reason I check is simple: Make sure the right plugin is calling this. After 5 years I am not really sure about my intentions there. About your Trac link: You might want to reconsider the problem you are facing there: Bulk uninstall is not working. Degrading the constant might not be the best solution. Imo there still should be a way to determine which plugin gets uninstalled.

comment How to create new category for custom post type?
Then please file an edit and add this explanation to the question. Aside from that, your code is incomplete and will throw errors. Also it is quite misleading to have add_games for registering a class_category taxonomy for a school custom post type.
