I have the opinion that, in general, forms are plugin territory. But hide/show a theme section is theme territory. So, you could have a plugin that handle the form, so you can reuse it across themes, and you could have a option in the theme to hide/show some part of the layout, with or without the form.

Currently, WP HTTP API uses cURL if possible; it not it uses default PHP functions for HTTP requests. WP HTTP API is server-side code, I can not imagine how HTTP sends information from the fornt end. Can you explain?

I've updated the answer, the code was wrong. Now it should work (I've tested and I use similar code in several websites). Unfornutatley I can not help you with Genesis as it is a third party software I don't know; try to test the code without Genesis to see if the problem is a conflict with Genesis or not.

Not sure if I understand your problem. Note that redirect is not the same that rewrite. A redirection means that the user is taken from one URL to another; the user enter a URL in the browser and when he/she is redirected the URL in the browser will change. The rewrite doesn't change the URL that the user sees in the browser but it is internally rewritten to something else server-side. Can you explain again what you are trying to do and where do you have the problem?

If you know, what is the question. You asked "I want to know what this is.". The WordPress/4.1.2 referrer indicates that it is using WP HTTP API, I think.

It seems that someone is loading an image from your website. There was a time when people was blocking such things at server level because it consumes your bandwidth to show the image in another website. Personally, I don't care about such thing and I think most of webmasters doens't care neither
