comment Limit Login Attempts BEFORE PHP is executed?
This isn't strictly what you asked for but have you looked at putting CloudFlare in front of the install(s)?

comment remove_action not working with procedural function
@PieterGoosen Wow good to know (on load order). The wrapping function is necessary to get earlier priority and still have the remove_action indicate the action to remove is part of the wp_head action? Do you have a reference that says the action needs to be removed before when using a child theme? Thank you!

@toscho I figured that was to distinguish between wordpress.com and the OSS version. Thanks for clarifying.

comment register_sidebar / dynamic_sidebar with sidebar id
Yes, everything works fine if I don't specify an 'id' parameter. I only added the 'id' parameter to get rid of debug error messages but this is clearly worse. It shows up in the backend under Appearance > Widgets and again is clearly 'registered'. But again dynamic_sidebar always returns false when I add an id parameter to register_sidebar.

No idea why but this did not work for me. I'm not sure if the suggestion was more about using __() or removing the other parameters but I copy and pasted this with no luck.

Tried some of the suggestions and added a note or two about the outcome.

comment Custom post type term names with ampersand in the term name
When I try to attach a term to a custom post type where the term name includes an ampersand WordPress finds the term fine (when doing the type ahead search) but because WP is not turning the & into & it never finds the term in the database when trying to attach it to the post. So I add the term in the admin edit post screen and hit update post and the post never has the term I just tried to attach when it finishes. The code I provided fixes it but I can't believe that's a proper fix. It holds the term name. Thank you!

comment Strange problem with wp_get_attachment_image_src database (db) query count
@TheDeadMedic That appears to reduce the query count by between 50-100 queries but if I comment out that the wp_get_attachment_image_src part it goes down by ballpark 300 queries. Seems really strange that manually specifying an integer post id fixes the problem.
