Ah - I'd assumed they shared a root, but hadn't looked into it. Cheers for the sharp and perfect answer :)

A theme can contain functionality too you know? And a theme doesn't mean a design.

Image sizes are generated on upload I believe. See answer below.

comment Auto activate plugin with unique user settings
Could your plugin manage its own settings instead of using the Wordpress settings manager? You could just generate the things you need and keep them in the plugins tables or something like that?

The user had the terms thumbnails and images confused which made the question lose its focus

Your thumbnail is being created at rustyrambles.com/wp-content/uploads/2012/02/… but I'm guessing you're saying you wish to use the large sized image, but it's not being genereated (i.e. rustyrambles.com/wp-content/uploads/2012/02/… doesn't exist). Did you make the media settings change after uploading the image, or before?

comment Putting my site live
That's exactly as I'd expect it to work, but I think anu was suggesting it was a more efficient way to put a site live by editing the config file. I've actually got one config file for live and dev sites with a switch statement switching on the domain name, name.com or name.dev (on my localhost) which sets the URL and database settings etc based on the url of the site.

@TomJNowell Well that's just not true I'm afraid. I run a number of sites with read only post types in the back end. As I've said numerous times, adding the read capability and removing the edit, delete and publish capabilites from user role provides EXACTLY this functionality.
