Is to complex for WordPress, you get a lot of problems with the core. Better is only plugins, content, uploads.

comment Making plugin unique to not conflict with plugins with the same name
Right, only via including source, like via plugin or theme. The easiest way is a other name, with crazy prefix ;)

comment How to use a TinyMCE filter?
What is the goal? You will remove default buttons and leave only bold and italic as button? You use the wrong hook, WP use 3 different hooks for each line in the TinyMCE, like mce_buttons, mce_buttons_2. See also this answer for more background and hints.

Then use a child theme or the hook, there I describe on the answer.

No the functions.php, the template file, where you will see the login link, like header.php

comment how to disabled a button when press three times
That's only possible, if you save the count, like in the database as option.

comment Reset Undo on the tinymce editor
On the WP default TinyMCE editor is the undo button include, inside the line 2 of the TinyMCE editor. You see this second line only, after click on "Toolbar Toggle" button, the last button on TinyMCE line 1.

comment Regionalised Content
I have not the answer for your question, but maybe you should think about a solution via WP Multisite. Each site in the network is a region. Is much better for capability management, templates, themes, plugins, domains and much more.
