Try to force your form to send data in UTF-8:<form accept-charset="UTF-8">. I think it's user's browser issue. Have you tried to replicate the problem in different browsers?

After update: It's impossible to force somebody to logout because logout happens in current user browser only (by deleting browser cookies). Session information is not stored somewhere else, e.g. database.

Have you cleaned browser cache? Or refresh that page using Ctrl+F5 after code change. Also, wp_logout destroys only current user session as it just cleans up cookies in user browser.

Have you tried my updated code? And also have a look into logout link url in both versions. It should contain .../wp-login.php?action=logout&redirect_to=...

@dave, I have no more ideas. The only I could do in place of you is temporarily alter each return one by one in get_page_by_title(), get_page() and get_post() functions in /wp-includes/post.php` to see which one exactly breaks the thing and why.

get_page_by_title() returns post' object or one of numeric and associative arrays, containing post information, not post id.
